Our staff at Polaris works closely with area hospitals like Kaiser Permanente South San Francisco and San Mateo Medical Center to ensure that we can assist with discharges from the hospital back to home. Often times during this process our clients need to spend a short period of time in a local skilled nursing or rehabilitation facility before heading back to their private home. In these cases, we work closely with your loved one’s physician, the social services and physical therapy departments at these facilities, such as San Bruno Skilled Nursing and Winston Manor Care Home. Working with the staff at these facilities allows for a smooth transition and continuation of therapy exercises after discharge.
