San Jose is home to over 125,000 senior citizens, many of whom will want to remain in their homes as they age. As our senior population ages, many will need extra assistance and long-term care. At Polaris Home Care, our caregivers are trained to provide in-home care in the San Jose area that allows for our clients to maintain independence in their own homes as they age.

We at Polaris Home Care work with the nearby hospitals and skilled nursing facilities to help with discharge services and transportation back to the home. We have worked closely with facilities in the area such as Good Samaritan Hospital, Kaiser Permanente San Jose, White Blossom Care Center and Mission Skilled Nursing and Rehabilitation Center to name a few. We ensure that all clients are as prepared as possible for their return home by working closely with the Physical Therapists and Social Workers at these facilities. In addition to in-home care, our Polaris caregivers are also able to work within assisted living facilities in the area such as Merrill Gardens at Willow Glen and Atria Willow Glen.

Our caregivers are able to provide assistance with activities of daily life upon their return home.  Assistance with personal care items such as bathing, and dressing assistance is provided.  In addition to personal care, our caregivers also assist with companionship services including meal preparation, shopping assistance and transportation.
